A Daihatsu station wagon was travelling south on Summerland Way, Dilkoon, failed to negotiate a slight right hand bend and left the roadway about 12.50pm Wednesday.
It struck a tree before falling down an embankment. Witnesses stopped and contacted emergency services.
The driver, a 73-year-old man, and the passenger, a 75-year-old woman were trapped for some time.
The woman died at the scene.
The driver was taken to Lismore Base Hospital with a fractured sternum. He was subject to mandatory blood and urine tests.
A report will be prepared for the Coroner.
