The 1-0 victory in first grade also ended a six match winless run against their rivals.
In the highly entertaining and even contest, Kurtis Smith's 71st minute penalty proved the difference.
Saints' midfielder Jarrod Witheridge provided a spark for his side as he made a run into the Rangers' box and was brought down by a defender.
Smith stepped up to the spot and made no mistake as he converted the penalty.
Following the goal, Rangers pressed further up the field in search of an equaliser.
However, they were almost caught on the break with six minutes remaining as goalscorer Smith was brought down in the Rangers box and another penalty was awarded.
Dan Baker had an opportunity to ice the game but Rangers goalkeeper John Clancy produced a miraculous save to keep his side in the contest.
Macleay Valley tried desperately to level the scores in the final moments but Saints' defence held firm.
Saints coach Kevin Sinclair was proud his side was able to earn the gutsy victory.
"It was a good, hard and physical game. It wasn't the best football from either side but the intensity and effort from both sides made it a great game to watch," Sinclair said.
"Neither side had a dead set sitter or any great chances but both keepers were worked over.
"It's always special to win a local derby but to also knock off the 2018 premiers makes it even sweeter."
Sinclair said it was a memorable day for Saints, as they achieved what no team had done before them.
"To hold the Rangers scoreless on their home ground in both grades was very special," he said.
"It says a lot about where we are defensively. To secure two clean sheets is outstanding and when you have a good defence, more often than not you will be on the right side of the score board."
While the victory serves as a confidence booster for Kempsey, Sinclair acknowledges it's still very early in the season.
"We won't be getting too carried away," he said.
"At the end of the day, a win is a win and there's a long way to go. There's still a lot to work on before we are where we want to be."
Saints captain Troy Ward, who was also awarded player's player at the conclusion of the match, produced a terrific performance.
"Troy was everywhere on Saturday," Sinclair said.
"The effort he showed to get across the park in attack and especially in defence was outstanding."
Sinclair also credited Sam Mahoney, who returned from his two week honeymoon on Friday but still made it onto the field.
"Sam partnered Troy as a defensive midfielder and was exceptional," Sinclair said.
The Kay Clarke Memorial Shield was also on the line and a $100 voucher was presented to Jake Conomos, who the Clarke family nominated as the player of the match.
As the winning club, Saints also won $300 and they chose to donate the winnings to local charity Baylin's Gift.
The victory lifts Saints into fifth position on the ladder and they still have a game in hand. Meanwhile Rangers' first loss of the season drops them to fourth position on the ladder.
