At 11.30pm, police said there was an altercation between two women, aged 20 and 24, in the carpark at the Nambucca Plaza.
The 52-year-old mother of one of the women arrived in her car at the scene shortly after and allegedly hit both - trapping them under her vehicle.
Police allege the driver then left the scene.
Emergency services personnel and members of the public managed to lift the car and free the women, but both died at the scene.
The deceased are understood to be a 24-year-old from Bellingen and a 20-year-old from Macksville.
The 52-year-old was arrested today after being treated in hospital.
She has been charged with manslaughter, dangerous driving causing death and failing to stop and render assistance.
She is to appear in Kempsey court today.
Police resources from across the State were called in to investigate the incident that has left two families reeling from the tragedy. Detectives from the Mid North Coast Police District are leading the investigation.
Officers have appealed for anyone who witnessed the incident to come forward.
"The circumstances surrounding this incident are horrific and have resulted in the tragic loss of two young women," Mid North Coast Police District Commander Superintendent Paul Fehon said.
ABC Local reported that police did not say which of the women was related to the driver or whether they were aware of any circumstances that may have led to the incident.
"We are looking at all the circumstances surrounding this incident on the evening and we're calling for members of the public who have witnessed this incident or who have any knowledge in relation to the circumstances to contact police," Supt Fehon said at a press briefing at Port Macquarie yesterday afternoon.
Police are also appealing for any information about a vehicle similar to a white Commodore in the area at the time of the incident.
Anyone with information is asked to call Crime Stoppers on 1800 333-000 or contact Kempsey police.
