Kempsey Saints Green claimed the minor premiership while the Macleay Valley Rangers locked up third position after securing a 1-0 victory over the Camden Haven Redbacks on Saturday.
Kempsey Saints White grabbed the last finals spot as they finished in fifth position.
Meanwhile Saints Green ensured they would head into the finals series on a high after earning a 7-1 win over the Upper Macleay Yowies.
The victory extended their winning streak to 10 games.
The side has the benefit of a week off as they finished on the top of the league and are one win from a grand final appearance.
The Macleay Valley Rangers will do battle with the Port United Strikers, with the winning side to advance to take on Saints Green.
Meanwhile the losing side will face the winner of Saints White and the defending premiers the Wauchope Waratahs, who face off in an elimination final.
